T-Shirts

Complete both forms below to purchase past festival t-shirts. 

• • First form is to select what shirts and sizes you'd like to purchase - You will get a confirmation email when your shirts are ready to be picked up in Dayton.

  • You may need to scroll using the far right bar to complete and submit the google form, then scroll again to get to the payment form

  • Please select the shirts you want in the google form and enter payment in the Boonshoft form (at the bottom of this page)

Second form for payment via credit card

• • • Invoice Number = 20241

    • Payer = Your name

    • Amount Due = Total # shirts x $5/shirt

    • Description = CWF Past T-Shirt order

    • Please select the box to cover the credit card fees

    • Lastly, complete the credit card information and once the form is submitted successfully a confirmation message will appear
